Our Changing Lifestyles Diabetic Educators and Registered Dietitians help further support and guide patients with nutrition and diet, and the services are provided by our sports physiologist to further help with those requiring monitoring for exercise or who have limitations for activity due to cardiac, pulmonary or other limiting health risks.

Frequently Asked Questions:

A referral from a medical doctor is required before patients can be booked in with the doctor. Referring doctors can download our referral form, fill it out and fax it to 970-625-6583 along with the patients’ latest blood work/note results to make the referral. Or, simply send us a referral with labs.

Once we receive the referral we will be contacting the patient directly in order to schedule an appointment. We schedule appointments in the order the referrals are received.

Once we receive the referral from the medical doctor we schedule patients for a one-on-one visit with Dr. Bitajian, which is generally covered under insurance if the referral is accepted.

​We also offer nutrition services to further support and guide patients with their diet, which are also generally covered by insurance. However, patients will have to check with their individual benefits for coverage information. 

Generally, follow up visits are on a monthly basis. However, it can be case-dependent. Patients with a more complex medical history, or if taking insulin, may require more frequent follow ups in the beginning as medications will likely require adjustments with the dietary changes. As patients get more accustomed to implementing the program in their daily routine, naturally, they will find that frequent follow ups are not necessary and therefore will check in once every few months for monitoring their blood work.
