Grand River Health is expanding its non-invasive vascular testing services by adding Exercise Ankle-Brachial Index (ABI) and Toe-Brachial Index (TBI) testing, providing physicians with an additional tool to diagnose Peripheral Artery Disease (PAD) in patients whose symptoms may not appear during standard resting exams.

Grand River Health has long offered resting ABI and TBI testing. The addition of exercise testing allows clinicians to identify circulation problems that may only become evident during physical activity, helping patients receive earlier diagnosis and treatment.

Peripheral Artery Disease occurs when plaque builds up in the arteries, reducing blood flow to the legs and feet. Left untreated, PAD can lead to pain, decreased mobility, poor wound healing and an increased risk of heart attack and stroke.

Many patients experience leg pain only when they’re walking or exercising, yet their resting vascular tests may appear normal. Exercise ABI/TBI testing helps us identify circulation problems that might otherwise go undetected, allowing us to develop an appropriate treatment plan before the disease progresses.

During the test, blood pressure measurements are taken in the arms, ankles and, when appropriate, the toes before and immediately after the patient walks on a treadmill. Comparing these measurements allows clinicians to evaluate how well blood is flowing to the lower extremities during activity.

The test may be recommended for patients who experience:

  • Leg cramping, aching or fatigue while walking or exercising (intermittent claudication)
  • Leg pain that improves with rest
  • Symptoms suggestive of Peripheral Artery Disease despite a normal resting ABI test
  • Long-standing diabetes or other conditions that may cause stiff, calcified arteries, making Toe-Brachial Index testing more accurate

Exercise ABI/TBI testing can also help physicians determine the severity of restricted blood flow and assess how PAD affects a patient’s ability to safely perform everyday activities and exercise.

The testing is performed by Grand River Health’s Respiratory Therapists and is non-invasive, covered by most insurance plans and requires a provider referral or order.
